Welcome to the Living Well Feedback Survey
Thank you for logging on to give your feedback. Only the first 400 responses will be accepted, one per household. To participate and receive $5 off an upcoming shopping trip, please have your Living Well magazine with you, as you will need to type in data from the mailing label on the back cover. This data is required to receive the $5 discount and must be exactly as printed.
Once you have completed the survey including the mailing label data, your information will be matched back to your shopper loyalty card/club and a $5 discount for a future trip will be loaded to it. Please allow 4 to 6 weeks for processing.
